On December 30, 2007, a fire occurred due to a condensate (natural gas liquid that is flammable) be released from a pressure relief valve (PRV) vent line which formed a puddle on the compressor unit skid and sprayed the engine.

An experienced rig up driver was asked to drive a similar (but different) truck than what he was used to. The winch controls on the new truck worked in the opposite direction from his old truck, so when he went to release the winch line, it instead tightened, breaking the line at the OSH knot.

Two Floorman were working in a sub base approximately 4 meters above the ground removing the Bell Nipple from the top of the Blow Out Preventor (BOP). One Floorman had attached his shock absorbing lanyard to the Bell Nipple and he was pulled down and fell to the ground. The other was knocked off balance and fell backwards. Both received soft tissue injuries and returned to work the next day.
