On the Uptegroove GC 1A well site a 21 bbl tank was being prepared to be cleaned. The tank had a two piece expanded metal top. A roustabout crew had just finished removing the bolts holding the expanded metal top on to give access to a vacuum truck to clean out 3" of mud in the bottom. A laborer on the roustabout crew placed his left foot on the expanded metal and his right foot on the cellar board.

While testing BOP's to 10,000 psi, a Lock Down Screw (LDS) was ejected from the wellhead and struck the Mouse Hole Casing. There was approximately 6000 psi on the Wellhead when the 5 pound LDS assembly came out.
